Summary of the Care360 EHR Grant Offer

• The offer period for the Care360 Grant Program began in January and ends December 31, 2012.
• Included in the Grant offer are Care360 EHR and Practice Management (PM)  licensing along with standard training. With this Grant, you can enjoy a fully-integrated EHR solution including system access, hosting, maintenance, training and 24/7 support. 
• The Care 360 EHR Grant program will subsidize 85% of the retail price of Care360 EHR and PM licensing upon  execution of the Grant and EHR License Agreements according  to these timelines:
  • A 15-month license, if contracts are fully executed (and first year payment made) between May 1, 2012 and August 31, 2012 or
  • A 10-month license, if contracts are fully executed (and payment made) between September 1, 2012 and December 31, 2012.
• Grantees will receive 85% off the retail price of training and implementation.
   

Eligibility Criteria for the Care360 EHR Grant Program 

• Eligible Grantees include any of the following located in the continental United States, Alaska, and Hawaii (except where restricted by law – see below):
  • Any physician practice (includes all physicians and physician extenders of that practice), but excluding hospital-owned physician practices; and/or
  • Any independent nurse practitioner office/practice
• The Program is not available in New York and Missouri due to state law restrictions
• The program is available in New Jersey subject to certain limitations
• The program is not available to  physician practices or nurse practitioners that already have EHR technology or had it in 2011

The Care360 EHR Grant Program Excludes:

• Additional EHR and PM services and modules (which includes practice management transaction fees, Patient Portal premium offering and Advanced Clinical Decision Support), additional third-party clinical interfaces and additional training services beyond standard training.
• Interfaces with third-party practice management systems.

• Grantee must pay any Grantee Fees as required by the Grant Agreement prior to the due dates. Failure to do so will nullify Grant offer made by Grantor.  
• Monthly subscription fees are available at 50% of full-time rates for licenses for part-time providers, nurse practitioners and physician assistants.

This offer is not, nor may it be, conditioned on doing laboratory business with Quest Diagnostics or any of its affiliates. The Care360 EHR Grant Program is designed to comply with applicable federal regulations, including the Stark Law, permitting an 85% subsidy of EHR solutions by commercial laboratories.

Will Quest Diagnostics support the use of laboratories other than Quest Diagnostics?

Yes. Care360 EHR offers access to universal lab orders and result capabilities, including a plug and play interface to Quest Diagnostics and optional interfaces to any other reference or hospital laboratory systems.

If I already have a practice management system, do I need to change it?

While the Care360 EHR Grant Program extends to include our integrated Practice Management capabilities, there is no need to change if you are happy with your existing practice management system.  We can interface to your existing system.  Retail rates apply.
